According to the New Jersey Division of Health Facilities Evaluation and Licensing, Burlington Woods was routinely inspected on two separate occasions between December 2008 and November 2010. During those inspections, Burlington Woods was cited for 12 separate deficiencies. It was found that the facility failed to make sure the nursing home was free of dangers that cause accidents.
